A hiatal hernia or hiatus hernia occurs when part of the stomach protrudes through the diaphragm and up into the chest. In an individual who has developed hiatas hernia, the opening of the esophageal hiatus (opening) is larger than in a normal individual, and a portion of the upper stomach slips up or passes (herniates) through the hiatus and into the chest.

Para-esophageal hernia: a type of hernia, in which the gastro-esophageal junction stays where it should be, i.e., attached at the level of the diaphragm, but a part of the stomach passes or bulges into the chest beside the esophagus.

Large para-esophageal hernias are dangerous and often require surgical intervention. During this type of hiatal hernia surgery, the stomach is pulled down into the abdomen, the esophageal hiatus is made smaller, and the esophagus is attached firmly to the diaphragm.

Treatment of sliding hiatal hernia is, in contrast, very simple. This type of hernia rarely causes problems themselves, but contributes to acid reflux, the treatment for patients with hiatal hernias is usually the same as for gastro-esophageal reflux disease (GERD). If the GERD is severe, complicated, or unresponsive to reasonable doses of medications, surgery is often performed.
